How to use the Questionnaire plugin

Create quizzes, surveys, and custom forms with scoring, logic, and plugin integrations.

The Questionnaire plugin is one of the most powerful tools in the marketplace. It allows you to create interactive surveys, tests, or forms that support tagging, scoring, conditional logic, and plugin integrations.


Requirements

  • This is an Accelerate plugin, available on specific plans only.


How to set up the Questionnaire plugin

  1. Log in, go to Builder in the left navigation

  2. Click the + button to open the Plugin Marketplace

    1. Search for Questionnaire, then click Add Plugin

    2. Name your plugin and click Add

  3. Stay in the Content tab:

    1. Enter a Title (this appears at the top of the questionnaire)

    2. Set a Maximum Score if using scoring

    3. (Optional) Turn on Shuffle Questions to randomize order

    4. Click + Add Question to begin creating questions. For each question:

      1. Choose a Question Type (e.g., single select, text, upload image)

      2. Add a Title and optional Description

      3. Add Answers (if applicable), along with:

        1. Optional image

        2. Score

        3. Tags (applied to users based on responses)

        4. Logic (e.g., skip to a specific question)

    5. Use optional settings to:

      1. Mark question as required or optional

      2. Add an Answer Template for logic or scoring summaries

      3. Make the question Private to hide it from others

      4. Drag and drop questions to reorder them

  4. Click the Design tab:

    1. Customize colors from your Branding tab or set manually

    2. Adjust Button Roundness

    3. Edit the Start Page:

      1. Add a background image or color

      2. Add text, video, or welcome content

    4. Edit the End Page (cannot be disabled):

      1. Add image, color, and completion content

    5. Choose a layout style or enable Advanced CSS Mode (for expert users only)

  5. Click the Settings tab:

    1. Under General:

      1. Require login

      2. Save answers to app data

      3. (Optional) Enable sending data to an external API

    2. Under Score & Review:

      1. Enable scoring, response summary, and pass/fail display

      2. Allow review of answers, show correct answers, and test duration

      3. Let users export their results

    3. Under Customization:

      1. Show questionnaire title, question count, or current question indicator

      2. Let users go back or skip questions

      3. Auto-load previous answers

      4. Allow users to resume at first unanswered question

      5. Show "Submission Complete" message or "Retake" button

    4. Under Time Constraints:

      1. Set time limits, availability windows, and cooldowns

      2. Limit submissions (e.g., 1 per user)

    5. Under Location:

      1. Enable map picker or manual location entry

    6. Under Action Items:

      1. Redirect users to another app section after submission

      2. (Optional) Trigger push notifications

  6. Go to the Security tab:

    1. Turn on Require Login (required for notes/bookmarks)

    2. (Optional) Restrict access by tag

  7. Go to the Tagging tab:

    1. Apply a tag to users when they visit this feature

  8. Go to the Purchase tab:

    1. Require a single purchase or subscription to access

  9. Click the Analytics tab:

    1. View data such as number of views, users, and devices over time

  10. Click the Results tab:

    1. View individual submissions with time, answers, and any uploads

  11. Click the Language tab:

    1. Customize field labels and prompts to match your app voice


Pro tips

  • Use the Answer Template feature to create logic-based reports or calculate totals

  • Apply Tags for automation: unlock features, assign badges, or trigger messages

  • Use Action Items to redirect users after completing a quiz (e.g., to a leaderboard)

  • Export results for offline grading or analysis
